Abstract: The randomized clinical trial paradigm is sketched, as well as the foundations on which its validity is based. Issues that jeopardize this validity, such as patient noncompliance and early withdrawal, are discussed. Intention to treat, being an important historical answer to this problem, is introduced, together with a set of criticisms and an indication of alternative approaches.
